Health History Form is a document used to collect an individual’s medical background, lifestyle habits, and relevant health details. It helps healthcare providers, fitness professionals, or wellness practitioners assess risks, customize treatment or programs, and ensure safety.

Par Q+

The PAR-Q+ (Physical Activity Readiness Questionnaire for Everyone) is a self-screening tool used to assess whether it’s safe for an individual to engage in physical activity. It’s often used before starting a fitness program or new exercise routine. The PAR-Q+ is a more comprehensive version of the original PAR-Q, designed to help identify if someone has a pre-existing health condition that may put them at risk during physical activity. It consists of a series of yes/no questions that ask about personal health history, including: Heart conditions (e.g., chest pain, heart disease). Bone or joint issues. Medications being taken. History of dizziness, fainting, or other symptoms. Recent surgery or injury. High blood pressure or other serious medical conditions. If an individual answers "yes" to any of the questions, it's recommended they consult with a healthcare provider before beginning an exercise program. This helps ensure their safety during physical activity. The PAR-Q+ is typically used by fitness professionals, coaches, or healthcare providers to make informed decisions about exercise participation.
